Wow! That just gave me a small idea for the fighting bosses part. Maybe in a realm name like "Dungeon" that belongs to one of the developers or mods, there could be something sort of like a dungeon door that leads to a separate areas where bosses spawn. And there could be small mobs, or Large bosses! The harder the boss the more chance you have a of it dropping a rare item that can only be obtained from fighting monsters. Also there could be different weapons (the better the weapon the more health points are taken away and the more experience points are given.) And lastly there could be different monster fighting levels (not regular levels but like surgery levels) and once again the higher your level the more chance of it dropping a rare item that is only obtainable from fighting.
